A 50-year-old woman presented with swelling and pain in the apical area of teeth #21 (9) and #22 (10). Additional exams confirmed an apical lesion in both teeth. The lesion is a recurrence of an apicoectomy done 3 years ago on the same teeth. The patient does not want to lose the anterior teeth which were rehabilitated 6 years ago, renewing a treatment with 4 crowns done 12 years ago by another dentist.
